Aortic regurgitation in rheumatoid arthritis necessitating aortic valve replacement.

چکیده
Four cases of patients with aortic incompetence secondary to rheumatoid arthritis are presented. All survived aortic surgery (two having bioprosthetic aortic valve replacement and two a homograft aortic root replacement). A review of the pathophysiology and pathology of this rare cause of aortic regurgitation is presented. A rationale for the choice of aortic valve prosthesis is discussed.

Rapid progression of aortic regurgitation was closely observed in a patient with severe seropositive rheumatoid arthritis. Rheumatoid changes within the aortic valve leaflets resulted in severe prolapse necessitating emergency operation. The postoperative course was satisfactory.

Valvular heart disease is a rare complication of juvenile rheumatoid arthritis (JRA), with most cases associated with polyarticular JRA. The aortic valve is most commonly affected, and valvular involvement occurs months or years after the onset of JRA. Reported cases of valvular heart disease in patients with JRA in a pauciarticular pattern are rare. We report a case of severe aortic insufficie...
